EDU.PH Assisted Information Modification (Manual Revision) Form


Introduction

The initial information about a particular EDU.PH registration were filled up during the registration process. These may be modified any time by visiting EDU.PH DNS Section of http://services.ph.net and clicking on "Information Modification" link. This will trigger a Verification Email to be sent to the current Admin or Technical Contacts of the domain. Any of the two may confirm the requested modifications for the modifications to be carried out.

Over time, the Admin and Technical contacts leave the employment of the institution. Sometimes, their e-mail addresses change or are deleted. Sometimes, they become hostile to the institution and do not reply to e-mails. When both the Technical and Administrative contacts are no longer reachable through their current (as found in our database) e-mail addresses or do not respond to our Verification Email, our normal modification process outlined above can not be used. This is the only time when this Assisted Information Modification (Manual Revision) should be used.

Instructions

  1. Fill up the manual revision form below. You will then receive an e-mail containing instructions together with the details of your requested modifications to your current registration.
  2. Submit the following:
    1. A letter duly signed by the school head requesting for the manual revision, the details to be revised and the telephone/telefax numbers of the school head for PHNET's verification. The letter should be mailed (via post), e-mailed as a GIF attachment, faxed, or delivered to PHNET's office. This letter must contain the details as found in the e-mail you receive from PHNET from the previous step.
    2. Pay for this manual revision service. Payment details are found in http://services.ph.net/payment.html.
  3. A notification will be sent to the domain's new administrative and technical contact after the modification has been made.

Domain Administrator - Information (Required)
The Domain Administrator will be registered as one of the persons authorized to request changes in you school's DNS registration. For this reason, we recommend that you appoint a permanent employee of your school as its domain administrator.

Technical Contact - Information (Required)
Because the Domain Administrator may not be a technical person, your school may appoint a Technical Contact who will coordinate with PHNET on your school's DNS registration. This contact person may approve modifications and changes in your domain information.
