DNS Hosting Modification Form 



  This form is for institutions which have availed of the PHNET DNS Hosting Service. Modifications to the DNS Hosting information are done through this form.


The following are the steps for modifying the PHNET DNS Hosting service details for a domain:
  1. Fill up the registration form below to generate a Request Form. The generated Request Form will be immediately e-mailed to the addresses of both the technical and administrative contacts, per our current records. Notice that the Form below does not have a section for DNS records. However, the generated Request Form you will receive has a section for the DNS records. It is in that Form that your Technical or Administrative contacts should put the DNS records.
  2. Either one of the Technical or Administrative contacts must reply to the Request Form we e-mailed. After we receive a reply, we will automatically update all the related DNS Hosting information.

Domain Administrator - Information (Required)
The Domain Administrator will be registered as one of the persons authorized to request changes in you school's DNS registration. For this reason, we recommend that you appoint a permanent employee of your school as its domain administrator.

The Domain Administrator's e-mail address. PHNET suggests that you consider using an e-mail service that is independent of an ISP. Doing so allows you to avoid inconveniences in your e-mail service if your school changes its ISP. Please do not use a Yahoo mail account because its settings can not be modified and our verification system may reject modification requests from Yahoo accounts.

Technical Contact - Information (Required)
Because the Domain Administrator may not be a technical person, your school may appoint a Technical Contact who will coordinate with PHNET on your school's DNS registration. This contact person may approve modifications and changes in your domain information.
